Referrals

Medical and allied health professionals are welcome to forward referrals via email to rebecca@mothernaturepsychology.com.au - I endeavour to make contact with all referrals within three days and offer an initial appointment in under two weeks. As a private service there is a gap fee of $119.

Medicare requires a referral to contain the following:

• Patient’s name, date of birth and address.

• The number of sessions the patient is being referred for.

• State that a Mental Health Treatment Plan has been created.

• Request the provision of psychological treatment.

• State an eligible diagnosis or symptoms. 

• A list of any current medications.

• Signed and dated and contains either the practice address or provider number.
